Booking a Custom Shade Appointment

Click on BOOK SHADE APPT.

Choose the date and time you would like to book.

Enter patient name, email , Dental Office name and click Book.

An email confirmation will be sent to the patient immediately and an email reminder 24 hours before the scheduled appointment.
